Step-by-Step
Admissions Overview
Initial Inquiry & Consultation
Your admissions journey begins with a brief consultation, where we gather essential information about your medical needs, level of care required, discharge plans (if coming from a hospital), and preferred accommodations.
Our team will explain available programs—Subacute Rehabilitation, Long-Term Skilled Nursing, Assisted Living, and Memory Care—and help identify the best fit.
Clinical Review & Approval
Once we receive your medical records, our clinical leadership reviews:
- Hospital notes and physician summaries
- Medication lists
- Therapy evaluations
- Current diagnoses and treatment needs
- Safety considerations or special requirements
This ensures that our team can meet your clinical and therapeutic needs safely and effectively.
Insurance Verification & Coverage Review
Our admissions specialists verify benefits with:
- Medicare
- Medicaid (NJ)
- Managed care plans
- Commercial insurance plans
- Supplemental policies
We explain coverage eligibility, therapy allowances, Medicare days, and any out-of-pocket expectations before admission— ensuring full transparency for residents and families.
Room Placement & Accommodation Options
Based on your care level and preferences, we review availability in:
- Private and semi-private suites
- Subacute Rehabilitation unit
- Long-Term Skilled Nursing neighborhoods
- Memory Care secured environment
- Assisted Living residences
Our goal is to place every resident in an environment that supports safety, comfort, and individualized care.
